457 Deferred Compensation Plan Catch-Up Provision Packet (Plan Number 300832)

This packet has information on the Age-50 and Pre-Retirement Catch-up provisions. To take advantage of the Age-50 Catch-up described in this packet, complete the 457 Deferred Compensation Plan Amount of Deferral Change Form below and send it to:

457 Deferred Compensation Plan Amount of Deferral Change Form (Plan Number 300832)

Use this form to make changes in the amount of your deferral to your ICMA-RC 457 Deferred Compensation Plan. Send your completed form to:

Employee Information Change Form (Plan Number 300832)

Use this form to make changes to you name or marital status. You may log onto the ICMA-RC web site to change your address, beneficiary, and funds and allocations. You may need to enter the City Plan Number, 300832. You may also call:

Emergency Withdrawal Packet (Plan Number 300832)

Provides information on taking an emergency withdrawal from your 457 Plan due to an "unforeseeable emergency." Forms and examples of Qualifying and Non-Qualifying Requests are included. Call the Pension Administration Division to discuss what documentation you will need to provide and to arrange an appointment to hand in your completed application.

Loan Packet (Plan Number 300832)

Provides information on taking a loan from your 457 Plan. However, you may apply for a loan online at the ICMA-RC website. Click on Manage My Account and Loans to apply for a loan or to estimate loan payments. You may have only one outstanding loan at a time and you may apply for only one loan during any calendar year.

Purchase Service (Plan Number 300832)

Provides information on using your 457 Plan money to purchase prior service in an eligible defined benefit retirement plan.

Recission

Use this form to rescind (cancel) your lump sum, rollover, or co-administrator transfer or rollover request.

NOTE: You must submit three copies of the Rescission form to the Pension Administration Division.
