News
On July 1, 2026, retiree supplemental life insurance premium rates are increasing from $0.56 per $1,000 of coverage to $0.82 per $1,000 of coverage. For more information, read our notice of this change.

If you are turning age 65 soon, please contact Medicare to apply for Medicare Parts A&B. Enrollment in Medicare Parts A&B makes you eligible to enroll in one of the City’s Medicare Advantage Plans with United Healthcare or Kaiser Permanente, which will lower your monthly premiums. Life Insurance
Forms
All forms should be submitted to the Department of Human Resources Benefits Division by mail at 2331 Mill Road, Suite 301, Alexandria, VA 22314 or by email at DHR.Benefits@alexandriava.gov
- Early Retiree FY 2027 Open Enrollment Forms
- Retiree Information Form (Fillable): Complete this form to update your address and contact information on file with the City
- Retiree Medical Insurance Reimbursement Statement ("the blue form"): Complete this form to request reimbursement for premiums you pay for non-City sponsored medical insurance
- Banking Form: Complete this form to allow the City to debit your bank account for health and/or life insurance premiums and/or credit your account for medical insurance reimbursements. NOTE: You must send a voided check along with this form.
- Life Insurance Retiree Beneficiary Form: Complete this form to change/update the beneficiaries of your City life insurance
- Life Insurance Termination Form: Complete this form to terminate your supplemental and/or dependent life insurance coverage
- Life Insurance Claim Form: If a retiree who had life insurance coverage under the City's policy has died, the beneficiary(ies) should complete this form and submit it and the retiree's death certificate to the Department of Human Resources at the address below.

Data in machine-readable files is updated monthly.
