How to Request Behavioral Health Treatment Services:
Services are available to residents of the City of Alexandria and offered through Same Day Access in-person or by tele health.
Please call 703.746.3535 for more information and to be screened for eligibility. Staff can answer calls in English, Spanish, and other languages through interpretation services available by phone when needed.
Same Day Access Tele Health and Walk-in Hours for Clinical Intakes:
Call 703.746.3535 or walk in during the following hours to begin the process:
- Telehealth: Monday through Friday, 9 a.m. to 2 p.m.
- Walk-ins: Monday through Friday, 9 a.m. to 2 p.m.
Please note that the above hours of operation are subject to change without notice and/or subject to clinician availability.
For tele health intakes, please have a smartphone or computer that includes internet connection, video, and audio capabilities, and a private space to talk with a clinician.

The following documents are required at the time of intake:
- A valid government issues photo identification (ID)- this includes a driver’s license, state issues photo ID, passport, military ID, or birth certificate.
- If you are a veteran or military service member, bring a copy of your DD214 or statement of military/veteran benefits.
- Proof of address with a date in the last 30 days- including recent utility bill, USPS change of address verification, deed, mortgage statement, lease agreement, official document, or correspondence from social services or other federal, state or local government agencies displaying name and current address, or billing statement from a recognizable business.
- Proof of Income- a recent pay stub dated in the last 30- 60 days, social security income statement, W-2 form, completed tax return or transcript.
- Copy of insurance card, if applicable.
- Proof of guardianship in cases where the designated primary client is not able to legally consent to treatment and a legal guardian has been appointed to make decisions on behalf of the client.
- Recent hospital discharge report/summaries, if applicable.
- Prescription medication list or bottles.
- Current or previous medical, psychiatric/mental health, and substance use provider records.
